School Bubble LLC Rider Agreement, Release and Consent

This agreement is between School Bubble LLC, a Florida company based in Cape Coral ("School Bubble"), and the parent or legal guardian who signs below ("Parent"), for school transportation of the children listed in this form.

1. Service. School Bubble drives your child between your home and school on the days and times you chose. School Bubble is a private service. It is not a school district bus and is not run by Lee County Public Schools.

2. Drivers and vehicles. Every driver passes a background check and holds a valid license. Every vehicle is insured for this use. Seat belts and the right car seat or booster are used on every ride.

3. Car seats. Florida law requires a car seat for children 3 and under and a car seat or booster for children 4 and 5. School Bubble provides boosters. The Parent provides the car seat for a child under 4 and leaves it with the driver. School Bubble may refuse a ride if the right seat is not available.

4. Pickup and hand off. Your child must be ready at the door at the planned time. The driver waits up to 3 minutes. In the afternoon the driver hands your child only to the adults you listed, and checks their face or photo. The driver never leaves a child alone.

5. No one home. If no authorized adult is home, the driver keeps your child safe in the vehicle, calls you and your emergency contacts, finishes the route and tries again. An extra fee may apply. If nobody can be reached, School Bubble may contact local authorities to keep your child safe.

6. Behavior. Riders stay seated with the belt on, keep hands to themselves and follow the driver. School Bubble may pause service for a child whose behavior puts others at risk, after talking with the Parent.

7. Health and emergencies. The Parent gives true and complete allergy, medication and health information and updates it. In an emergency, School Bubble will call 911 first and then the Parent. The Parent authorizes emergency medical care for the child if the Parent cannot be reached, and is responsible for its cost.

8. Location and privacy. The vehicle location is shared with the Parent only while the child's route is active. Photos, school and health details are used only to run the service and are seen only by the child's own parents, the assigned driver and the School Bubble office. School Bubble never sells family information.

9. Price, payment and cancellation. The Parent pays the weekly price before the week of service. Tell us by Thursday to skip or stop the next week. Days missed for school closures, weather or holidays are not charged when we know at least the day before. A week already paid ahead is not refunded for days the child does not ride without notice.

10. NOTICE TO THE MINOR CHILD'S NATURAL GUARDIAN: READ THIS FORM COMPLETELY AND CAREFULLY. Riding in any motor vehicle has risks that cannot be fully removed, even when School Bubble uses reasonable care. By signing, you release School Bubble LLC, its owners, drivers and staff from claims for injury or property damage caused by those inherent risks of road travel. This release does not cover harm caused by School Bubble's negligence, gross negligence or intentional acts, and does not limit any insurance coverage. You have the right to refuse to sign this form, and School Bubble has the right to refuse to transport your child if you do not sign it.

11. Changes. The Parent tells School Bubble right away about any change in address, school, schedule, authorized adults or health. School Bubble may update this agreement with 14 days notice by email or text.

12. Florida law governs this agreement. The Parent confirms they are the parent or legal guardian of each child listed, that the information in this form is true, and that they signed it electronically.
